Doctors ask White House to send back FDA's peptide plan

Take

Until OMB acts, we wouldn't sign a peptide supply contract or buy a peptide ad. The plan rests on 8-6 advisory votes. Organized doctors now want OMB to reject it, and OMB has no deadline. We think waiting a few weeks loses very little, while building now is a risky bet.

Doctors for America asked OMB on Friday to return FDA's "Interim Access Policy for Compounding Using Certain Peptides." It also wants OMB to clear no enforcement-discretion policy for a substance FDA hasn't put in Category 1. The group names BPC-157, TB-500, KPV, MOTS-c, Epitalon and Semax as the peptides at issue. Its sharpest point is about FDA's own staff: "In July, FDA's own scientists recommended that none of the seven peptides reviewed be added to the compounding list." The advisory committee overrode them 8-6, and their vote is only a recommendation. DFA adds that one newly appointed committee member runs an anti-aging clinic that sells a BPC-157 product for $149.

Meanwhile, the American Academy of Peptide Medicine holds the only meeting on the calendar. It has booked time with reviewers Wednesday at 3 p.m., per OIRA's meeting log. The plan's text isn't public. The only account of what it would allow came from AP last week. Pharmacies could compound popular peptides such as BPC-157 for a while without FDA coming after them. That is enforcement discretion, and FDA can take it back as easily as it grants it.

The review record still says Pending Review. Its legal-deadline field reads "None."

One in nine large employers has dropped GLP-1 weight-loss coverage or is weighing it

Employers settle next year's drug coverage this year, so cuts show up in surveys long before a patient sees a denial. Mercer asked 481 large employers this spring. Six percent had already dropped GLP-1 weight-loss coverage for 2026. Another 5% planned to drop it in 2027 or were seriously considering it. And 27% had tightened limits on use or planned to. Vox names PepsiCo, Starbucks and Lowe's among the brands cutting back. The odder number comes from Massachusetts, where commercially insured GLP-1 users fell by half between 2025 and the first quarter of this year. Many of those patients will try to pay cash. In January they will compare your monthly all-in with LillyDirect's $299 starting price on the board below.

In a photo study, 54 of 83 compounded GLP-1 users said they were on a brand

University of Michigan researchers had 322 weekly semaglutide and tirzepatide users upload a picture of the label. Coders classed 83 as compounded, Medscape writes. The authors warn that "reliance on self-report alone may not reliably classify medication type." Nobody checked the photos against lab tests. Clinics that take transfer patients can ask for the same photo with one upload field.

FDA inspectors found research-only ingredient paperwork at a pharmacy that just recalled tirzepatide

Medical Daily reports that Houston RX recalled 64 vials of compounded tirzepatide in September for a lack of assurance of sterility. An August FDA inspection there produced a Form 483 with 14 observations. The certificates for its tirzepatide and semaglutide ingredients were marked for research and development only. A 483 lists what inspectors saw, and FDA hasn't made a final finding. USA Today covered a second tirzepatide recall from Apothecary Pharma. FDA sent that company a warning letter last December. Before your next vial order, ask your pharmacy for its most recent inspection report.
